Walkthrough of the reporting form

Key points

  1. In the lesson, we went through the reporting form developed to capture progress of the project according to the M and E framework we have seen in the previous lesson.
  2. The reporting form is designed to be filled out at the end of each mobile clinic, capturing essential data to track project progress based on the M&E framework.
  3. The form includes fields for date, district, and gender to enable disaggregation of the collected data.
  4. The form has sections for each indicator, such as the number of youths who collected condoms, number of condoms collected, number of youths who accessed HIV counseling and testing services, and number of youths who accessed voluntary medical male circumcision.
  5. To calculate the total number of mobile SRH clinics conducted, the number of submitted forms can be counted, as each form represents one clinic session.
